Referral Form for Sleep Testing

The attached file can be used for referral for sleep testing at the Rocky Mountain Sleep Disorders Center.   You do need to have a physician's signature to be scheduled for testing procedures.  

 If you do not have a primary phycian you may consider scheduling an appointment with our sleep specialists and they can then in turn determine if sleep testing is needed and order the procedures.   

Please check with your insurance provider to verify if appointments with the Sleep Specialists would be covered without a referral from your primary physician.
