Using the scale below, simply choose the appropriate number that applies to you in each scenario:
0 = Would never doze
1 = Slight chance of dozing
2 = Moderate chance of dozing
3 = High chance of dozing

___ Sitting, inactive in a public place (i.e., movie theater)
___ Sitting talking to someone
___ As a passenger in a car for an hour without a break
___ Sitting quietly after lunch without alcohol
___ Lying down to rest in the afternoon if time permitted
___ In a car, while stopped for a few minutes in traffic
___ Total Score
If your total score is higher than 10, it may indicate excessive daytime sleepiness. You should discuss this with your physician to determine whether you may have a sleep disorder.
